This appointment establishes you as a client, involves discussion of an extensive intake questionnaire, and allows us to set goals and order any necessary functional testing.

Cancelation Policy

Cancellation is required 24 hours in advance to avoid a $50 rebooking fee. Please understand that a missed appointment could have gone to another client, and preparation and study of your file in advance of your appointment takes considerable time.
